Documents and Forms
During professional practice, candidates and supervisors must use the standard documentation approved by IEKA. All forms and templates are published in this section and updated whenever IEKA's competent bodies approve changes.
Documents being updated
The templates below are being updated and will be published in this section shortly:
- The individual employment contract with the practice supervisor;
- The candidate's notarised declaration;
- The request to change the professional practice supervisor.
Until they are published, these documents can be requested at IEKA's offices.
